Fun gifts!
Brides can be very emotional about their weddings and it's a Maid of Honor's primary responsibility to help the Bride through it and keep her calm and happy. On the other hand, the Maid of Honor is a person with feelings too who has her own life and priorities. If you intend to become pregnant, suddenly become pregnant, or are already pregnant, you may prefer to keep this information to yourself, but don't be too surprised if the Bride reacts badly once she finds out.
As the Bride is likely a close friend of the Maid of Honor, but also potentially easy to upset during her wedding planning, I strongly recommend telling the Bride if you're pregnant or trying to have a baby. If you aren't comfortable telling the Bride right away, at least try to fill the Bride in as soon as possible since she'll eventually figure it out anyway. Surprises are generally bad for Brides!
Once the Maid of Honor is ready to talk, it's a good idea to gently tell the Bride over lunch or a casual chat. Emphasize how excited you are about this new phase of your life. The Bride may be getting married, but the Maid of Honor having a child is important too.
Any Bride who's a good friend should understand and be happy for you. On the other hand, as a pregnant or soon to be pregnant Maid of Honor, you should be willing to step down if the Bride asks it. It's not fair and I don't think Brides should do it, but being a Maid of Honor is stressful enough without having the Bride freaking out over you having a bit of a belly!
In practical terms, once you've filled the Bride in on the good news, it's time to think about the Maid of Honor dress and the Maid of Honor's event planning responsibilities.
If you're only going to be a "little" pregnant at the time of the wedding, just make sure that the Maid of Honor dress isn't altered so that it's too tight over the belly. Insure there's a bit more room there than you would normally need.
If you're going to be much more pregnant on the Bride's wedding day, have a discussion with the Bride about choosing a Maid of Honor dress different from the other Bridesmaid dresses (which is a normal thing to do anyway). You might want to recommend darker colors (if possible) and browse maternity Maid of Honor dresses for good style ideas.
Finally, if you're going to be a pregnant Maid of Honor, it's important to get sufficient help planning the Bridal Shower and Bachelorette Party. Pregnancy can be a wonderful time in a woman's life, and a pregnant Maid of Honor should make sure she reserves enough time to enjoy her own exciting life change.
